Processing Method Spectrophotometry
Overview: Alkaline Phosphatase Test
Introduction: The Alkaline Phosphatase Test measures an enzyme to evaluate liver or bone health, affected by conditions causing jaundice, abdominal pain, or bone weakness. Following 2023 clinical biochemistry guidelines, it uses spectrophotometry for high accuracy, aiding in diagnosing liver or bone disorders. This test is critical for guiding treatments, preventing complications, and improving outcomes in liver and bone health.
Other Names: ALP Test, Alkaline Phosphatase Assay.
FDA Status: Laboratory-developed test (LDT), meeting biochemistry standards for diagnostic accuracy.
Historical Milestone: ALP testing began in the 1930s for liver disease. Spectrophotometry emerged in the 1970s, and by the 2000s, automated assays improved clinical utility.
Purpose: Diagnoses liver or bone disorders, guides therapy, and monitors disease in patients with jaundice or bone pain.
Test Parameters: 1. Alkaline Phosphatase
Pretest Condition: Fasting for 10-12 hours is required. Collect serum or plasma at any time. Report jaundice, bone pain, or medications affecting liver/bone health.
Specimen: 2-5 mL serum (SST) or 2-4 mL plasma (EDTA). Centrifuge within 1 hour. Transport in a biohazard bag within 8 hours.
Sample Stability at Room Temperature: 8 hours
Sample Stability at Refrigeration: 7 days
Sample Stability at Frozen: 6 months
Medical History: Document jaundice, bone pain, or family history of liver/bone disorders. Include current medications or alcohol use.
Consent: Written consent required, explaining the test's purpose, diagnostic implications, and potential therapy needs.
Procedural Considerations: Uses spectrophotometry to measure ALP levels. Results are available in 1-2 days, supporting rapid diagnosis.
Factors Affecting Result Accuracy: Liver or bone disease may elevate ALP non-specifically. Hemolysis or improper storage can degrade samples.
Clinical Significance: Elevated ALP suggests liver or bone pathology, prompting further testing or therapy. Normal levels may require monitoring in chronic conditions.
Specialist Consultation: Consult a hepatologist or rheumatologist for result interpretation. Orthopedic referral is advised for bone issues.
Additional Supporting Tests: Liver function tests, bone-specific ALP, or imaging to confirm liver/bone pathology.
Test Limitations: Non-specific ALP elevation requires clinical correlation. Early disease may not elevate levels significantly.
