Overview: Gram Stain Test
Introduction: The Gram Stain Test detects bacteria in specimens to diagnose infections, causing fever or pain. Aligned with 2023 IDSA guidelines, it uses microscopy for high specificity, aiding in infectious disease screening. This test is critical for guiding diagnosis, antibiotic selection, and improving outcomes in bacteriology for patients with suspected bacterial infections. It differentiates Gram-positive and Gram-negative bacteria, guiding targeted therapy.
Other Names: Gram Stain Assay, Bacterial Stain Test.
FDA Status: Laboratory-developed test (LDT), meeting bacteriology standards for diagnostic accuracy.
Historical Milestone: Gram staining was developed in 1884 by Hans Christian Gram. Microscopy advancements in the 20th century improved its diagnostic utility for infections.
Purpose: Diagnoses bacterial infections, guides antibiotic treatment, and monitors bacterial morphology in patients with fever or pain.
Test Parameters: 1. Bacterial Morphology
Pretest Condition: No fasting required. Collect swab, sputum, body fluid, tissue, or pus during active symptoms. Report symptoms like fever or pain, and list medications.
Specimen: 1-2 swabs (sterile swab/transport medium), 5-10 mL sputum, body fluid, pus (sterile container), or 0.5-2 cma³ tissue (sterile container). Transport in a biohazard bag within 2 hours.
Sample Stability at Room Temperature: 2 hours
Sample Stability at Refrigeration: 24 hours
Sample Stability at Frozen: Not frozen
Medical History: Document fever, pain, or history of bacterial infections. Include current medications, especially antibiotics, and recent infections.
Consent: Written consent required, detailing the tests purpose, diagnostic implications, and potential need for antibiotic therapy.
Procedural Considerations: Uses microscopy to identify bacterial morphology (Gram-positive or Gram-negative). Results are available in 1-2 days, enabling rapid clinical decisions.
Factors Affecting Result Accuracy: Improper sample collection, contamination, or prior antibiotic use can affect results. Low bacterial load may yield false negatives.
Clinical Significance: Identification of Gram-positive or Gram-negative bacteria guides antibiotic selection. Negative results may require culture or molecular testing.
Specialist Consultation: Consult an infectious disease specialist for result interpretation and treatment planning.
Additional Supporting Tests: Bacterial culture, sensitivity testing, or PCR to confirm infection and guide therapy.
Test Limitations: Limited to detecting bacteria visible by microscopy. False negatives may occur with low bacterial load or prior antibiotics.
